Spoilers about the fic below:
My last picture of Fluttershy based on EQ:TW she was quite unhappy as throughout the fic Fluttershy is reluctant to take part in the battles, even as just a medic (She believes nopony should be fighting in the first place). However as the war progresses she begins to see that her actions will lead to more deaths if she does nothing so she reluctantly takes part in saving the wounded. This picture is set after she has this realisation (Hence the happy face) and after the mane six becomes friends once again (They have huge personality changes as the fic progresses which messes up their friendship).
Anyways check out the fic (Even though it seems to have died). It is still worth a read for what its worth.

I wouldn't mind having Fluttershy for a medic in the ambulance I work on. Some of the medics can be total jerks. Fluttershy and I would get along great!
